What is Endoscopic CRE Balloon Dilation?
Endoscopic CRE Balloon Dilation is a minimally invasive procedure used to treat selected narrowing, or strictures, in the esophagus, stomach, and intestines.
CRE stands for Controlled Radial Expansion. A specialised balloon catheter is passed through the endoscope and positioned across the narrowed segment. The balloon is then inflated in a controlled manner to gradually widen the stricture. Many CRE balloons are designed to provide three progressively larger diameter options, allowing the doctor to select the appropriate expansion for the individual stricture.
Used for Selected Digestive Tract Strictures
Your doctor may recommend CRE balloon dilation depending on the cause, location, length, and severity of the narrowing, as well as your symptoms and overall condition.
Esophageal Strictures
May be used for benign narrowing caused by chronic acid reflux, radiation therapy, post-surgical scarring, or injury after corrosive ingestion.
Swallowing Difficulty
Can help relieve dysphagia from selected structural causes such as Schatzki rings and other benign narrowing. Achalasia may require a different type of pneumatic dilation depending on the case.
Gastric Outlet Narrowing
Selected narrowing of the pylorus or duodenal bulb caused by peptic ulcer disease or corrosive injury may be treated with endoscopic balloon dilation.
Bile Duct Strictures
During ERCP, balloon dilation may be used in selected biliary strictures as part of endoscopic treatment.
Small Bowel Strictures
In selected patients with Crohn’s disease, endoscopic balloon dilation may be considered for short, accessible strictures.
Improve Passage of Food
The goal is to relieve symptoms such as difficulty swallowing, food sticking, chest discomfort related to narrowing, or impaired passage through the digestive tract.
What Happens During the Procedure?
The amount of dilation is selected according to the characteristics of the stricture. The doctor widens the area gradually rather than stretching it beyond what is considered safe.
Sedation & Endoscopy
Sedation is commonly used depending on the procedure. The endoscope is advanced to the narrowed area so the doctor can assess the stricture directly.
Balloon Positioning
The CRE balloon catheter is passed through the endoscope and positioned accurately across the narrowed segment.
Controlled Expansion
The balloon is inflated to a precise pressure and gradually expanded to the diameter selected by the doctor. CRE balloons may provide three diameter options within one balloon.
Deflation & Reassessment
After a short period, the balloon is deflated and removed. The treated segment is then reassessed endoscopically.
The Procedure is Designed to Be Well Tolerated
CRE balloon dilation is often performed under sedation, which helps reduce anxiety and discomfort. Some patients remember very little of the procedure, while others may recall parts of it. The experience varies according to the location of the stricture, the type of sedation used, and the individual patient.
What Should You Expect After Dilation?
You will usually be observed for a period after the procedure. If sedation was used, you may feel drowsy for a while during recovery.
Temporary Discomfort
If the esophagus was dilated, you may experience a mild sore throat or chest discomfort. Mild bloating can also occur and usually settles quickly.
Diet
You may be advised to begin with liquids or soft foods before gradually returning to your normal diet according to your doctor’s instructions.
Multiple Sessions
Depending on the cause, length, and severity of the stricture, more than one dilation session may be needed to achieve the desired result safely.
What Should You Watch for?
Tell your doctor about all medicines you take, especially blood thinners or antiplatelet medicines. Follow fasting instructions carefully before the procedure. Seek urgent medical attention if you develop severe or worsening chest or abdominal pain, fever, vomiting blood, repeated vomiting, difficulty breathing, significant bleeding, or sudden severe illness after dilation. Keep all scheduled follow-up appointments.
A Minimally Invasive Endoscopic Treatment for Selected Strictures
Endoscopic CRE Balloon Dilation can provide significant relief from swallowing difficulty and other symptoms caused by selected gastrointestinal strictures, while avoiding surgery in some patients. If your doctor has recommended this procedure, the treatment plan will be based on the cause, location and severity of the narrowing, as well as your overall health.
